Clinical Nurse (THHS Single Assessment Organisation), Queensland HealthTownsville Hospital and Health Service Single Assessment Organisation are seeking an experienced and client focused Clinical Nurse to join their multidisciplinary team.There are two (2) permanent full-time roles available.The Clinical Assessor provides advisory clinical support, education and information to staff, service providers, external/internal stakeholders, clients and carers.About the opportunityTo provide clinical and non-clinical home-based face to face assessments of clients referred via the My Aged Care Gateway using the Integrated Assessment Tool.
Under the Single Aged Care Assessment System, Aged Care Assessors work with clients and their representative/s to establish goals specific to the client's needs and develop appropriate strategies and actions, including referrals to aged care and non-aged care services.The Townsville Hospital and Health Service Single Assessment Organisation is a multi-disciplinary team providing assessment services to older Queenslanders in the community and the hospital setting.
