Formed in 2000, Anglicare North Queensland Limited (Anglicare NQ) is a for purpose not for profit Australian Charities and Not-for-profits Commission (ACNC) registered charity and an independent legal entity operating as a Company Limited by Guarantee. About Non-Family Based Care (NFBC)

Within Anglicare we support people at many different ages and stages in life. In the NFBC space, Anglicare NQ is using the 'Track my way' program, in this program, the Residential Care and the Supported Independent Living programs are linked. This provides a pathway for young people aged 12-18 in the out of home care space, leading towards independent living. Anglicare NQ provides a home, from where staff support young people to their next step towards independence, this may look different for each child, but the aim remains the same. Our program strives to ensure that the young people within our care are strongly connected with community, culture, and family, develop independent living skills and can use their 'intentional use of self', self-reflection, self-regulation, and anything else that might be part of raising children; waterslides, fishing, doing the dishes together, reading stories and so on.

About the role

As a Case Manager, you will take a holistic approach to supporting a case load of clients, addressing a broad range of needs and issues. This will include conducting assessments of clients' situations, identifying strengths, weaknesses, and needs. Based on these assessments, you will develop comprehensive care plans, outlining specific goals and interventions to address goals and needs. Implementation of plans may involve connecting clients with appropriate resources and services, such as healthcare, housing, employment, and social services. You may advocate for clients with other stakeholders, ensuring they receive the necessary support. Finally, you will regularly monitor, evaluate, and document the progress of clients, adjusting the care plan as needed.

To Be Successful You Will Need

* Proven case management experience using a holistic and client centered approach to managing your case load.
* Extensive experience working in government funded programs with strict budgetary and contractual requirements of service.
* Ability to build and maintain relationships with stakeholders, including government bodies, community organisations, and other service providers to provide efficient and effective services to clients.
* Knowledge of organisational programs, policies and operations.
* Industry relevant qualifications or relevant industry experience.
* Experience working with Aboriginal and Torres Strait Islander people, families and communities will be highly regarded
* Experience working with people from culturally and linguistically diverse (CALD) backgrounds will be highly regarded

Compliance

* A current QLD Drivers Licence 'Class C' or interstate equivalent.
* Blue Card (Working with children card)
* A current national police check (within the last 3 months)
* Current First Aid & CPR certificate
* CurrentLCS2 check, (Child Safety and Personal History ScreeningCheck)
