Support Worker in Child and Adolescent Mental Health Services
This role involves working collaboratively with a multidisciplinary team to provide patient-centric care, focusing on fostering hope for recovery through shared experiences. The Support Worker will be utilized across all stages of care, from initial consultation and care planning procedures to post-discharge follow-up, ensuring seamless transitions between acute mental health care and community services. The position holder will work closely with psychiatry and mental health practitioners to identify areas for improvement, ultimately reducing presentations to emergency departments.
To achieve these goals, the Support Worker will employ assertive support strategies, building strong relationships with consumers, families, and caregivers.
