Key Responsibilities and Objectives
* Provide empathetic pre- and post-placement support and guidance for families and care teams, while conducting comprehensive assessments of clients' needs, abilities, and behaviours to inform tailored care plans.
* Collect valuable data to ensure seamless transitions for clients with dementia from acute care to residential or home settings, thereby enhancing understanding of their social history and support networks.
Essential Qualifications and Skills
* Experience in Social Work or registered Psychology is essential
* Strong experience and knowledge in dementia care and mental health for older persons
* High level of communication and interpersonal skills to lead and influence
* Excellent problem-solving and decision-making skills
