Community Nursing Role
We are seeking an experienced and compassionate Enrolled Nurse to provide high-quality care to seniors, adults living with a disability, and adults recovering from a hospital stay in a community setting.
The ideal candidate will possess excellent communication skills, be able to work autonomously and within a team environment, and have exceptional time management and organisation skills.
* Completing client assessments, clinical care, and reviews
* Interpreting health, aged, and disability care policies and regulations for caregivers and nursing staff
* Ensuring compliance with local, state, and federal regulatory agencies related to aged and disability care services
* Implementing and maintaining operating systems, including Visual Care, policies, and procedures
* Ensuring caregiver management meets best practice standards, including ongoing professional development of the care team
* Serving as a liaison with accounts department, ensuring proper billing in line with the care plan
* Compliance with OH&S regulations
* Meeting with clients and their families, assessing needs, and building strong connections
* Conducting client care assessments and preparing care plans to meet client requirements
* Providing nursing care and in-home care services as needed
* Handling information requests, feedback, and general enquiries
* Ensuring accurate and comprehensive documentation in databases and software programs
Requirements
* Enrolled Nurse (EN) with 1-2 years' experience in community nursing and current AHPRA registration
* Certificate III or IV in Aged/Disability Care with minimum 4 years' experience
* Proven experience in aged care, disability, or mental health within a community setting
* Strong understanding of Aged Care and Disability Standards, consumer-directed care principles
* Current First Aid, CPR certification, National Police Check
* Working With Children check or willingness to obtain
