Support Practitioner Role
We are seeking a Support Practitioner to provide compassionate care and support to individuals with disabilities.
* Support clients to develop and maintain independence and advocate for their individual needs.
* Deliver personalised support by assisting clients at mealtimes, personal care, administering medication, manual handling using equipment and aids.
* Transport clients in wheelchair-accessible vans and pool cars.
* Monitor the well-being of clients and report on their progress.
The ideal candidate will have:
* Demonstrated commitment to supporting and promoting the individual needs of people with disabilities.
* Flexibility to work on a 24/7 rotating roster, including morning, afternoon, and active night shifts, as well as weekend and public holiday shifts.
* Experience supporting customers with intellectual disability, high physical support needs, and medical/health needs.
* A current NSW Driver's License (P2 minimum).
This role offers an attractive hourly rate + Super + Penalty Rates, a $100 per year Well-being Allowance, discounted gym membership, Employee Assistance Program, and opportunity to complete a nationally recognised qualification.

Key Responsibilities
Support Services

* Monitor client progress and update records accordingly.
* Report any changes or concerns to management.
