The role of a clinical pharmacist in subacute care is to provide safe and effective medication management for patients.
Working closely with the Director of Pharmacy, you will be responsible for assessing and advising on medication regimens, identifying potential issues and opportunities for improvement.
This requires strong clinical skills, attention to detail and excellent communication abilities. Key qualifications include registration as a pharmacist with AHPRA, a minimum of 4 years' experience in a community setting and proven expertise in supplying or supporting Aged Care Pharmacy Services.
A high level of verbal and written communication skills, interpersonal and professional organisational skills are also essential for success in this role.
You will work collaboratively with healthcare professionals to ensure seamless patient care, referring complex clients to appropriate services when necessary.
Key Requirements
* Registered as a pharmacist with AHPRA
