About the Role
We are seeking a compassionate and dedicated Social Worker / Case Manager to join our team in Bendigo. In this role, you will be an integral part of the Veteran Pathways leadership team, delivering essential social work services to veterans. You will focus on enhancing their physical, psychological, and social functioning, ensuring that their health goals are met with the support they need.
Your key responsibilities will include:
* Providing social work services to veterans in line with Department of Veteran Affairs (DVA) policies.
* Managing telehealth assessments, case formulation, treatment planning, case management, and reviews.
* Offering internal and external referrals and assisting with allied health intake bookings.
* Completing all documentation and reporting to prescribed standards and timelines.
You will also be responsible for:
* Developing and implementing psychosocial treatment plans, addressing issues such as anger management, family violence, homelessness, and rehabilitation.
* Ensuring the delivery of services aligns with the Pro-Active Health procedures and DVA guidelines.
* Contributing to continuous improvement initiatives and fostering a collaborative working environment.
About You
To succeed in this role, you will bring:
* A Degree in Social Work and current membership with the Australian Association of Social Workers (AASW).
* Experience working with veterans or a similar client group, with a focus on holistic care and chronic condition management.
* Strong communication and case management skills, with the ability to work both independently and within a team.
* A commitment to professional development and adherence to the AASW Code of Ethics.
* A compassionate approach to supporting clients, always prioritising their dignity, self-determination, and well-being.
You will also be expected to engage in regular supervision, participate in professional development, and uphold the standards of practice required by the AASW and Pro-Active Health.
