Social Worker, Community Care
About the role:
We are looking for an experienced Social Worker to join us in this exciting role to provide quality services to patients/clients of the Murrumbidgee Local Health District in order to meet their health and wellbeing needs
As the Community Care Social Worker, you will be responsible for:
* Providing clinical services to clients living with chronic and complex conditions in the community – you will demonstrate advanced practice skills and will operate as part of the integrated multidisciplinary care team.
* Exercising independent professional judgement when required to assist with the management of complex issues and management of cases where principles, procedures, techniques and methods require expansion, adaptation or modification.
* Working collaboratively with the wider multidisciplinary team
* Provide support to Allied Health Assistants and students as required

Position Classification: Social Worker – Level 3
Remuneration: $116, $120,680.00 per annum, pro rata (+ super +17.5% leave loading where applicable)

Our ideal candidate will demonstrate:
1. A Bachelor Degree in Social Work
2. Minimum three years post Graduate clinical experience
3. Eligibility for membership with the Australian Association of Social Workers (AASW)
4. Extensive knowledge, clinical proficiency and ability to work with consumers with late stage degenerative and chronic conditions and disability in the community.
