The Hills Community Health Centre is committed to delivering high-quality, person-centred healthcare to the local community. You will join a supportive multidisciplinary team dedicated to improving health outcomes for individuals living with chronic and complex conditions. Whilst this position is primarily based at The Hills Community Health Centre, there may be occasions where travel to other centres across the Western Sydney Local Health District is required for the management of the workload.

What you'll be doing
As a Chronic & Complex Endorsed Enrolled Nurse, you will provide safe and effective nursing care under the supervision of a Registered Nurse while supporting individuals to achieve the best possible health outcomes. You will play an important role in monitoring client health, delivering care in line with established plans and contributing to quality clinical practice.
· Deliver direct nursing care in accordance with individual care plans
· Monitor client observations, recognise clinical changes and escalate concerns
· Support clients with activities of daily living and promote independence
· Maintain accurate clinical documentation and health records
· Collaborate with the healthcare team to support safe, high-quality care

Essential Criteria:
· Registration:Current Endorsed Enrolled Nurse division 2 Australian Health Practitioner Regulation Agency (AHPRA)

1. Relevant clinical experience in speciality.
2. Demonstrated ability to organise and prioritise work.
3. Demonstrated effective communication skills both written and verbal.
4. Effective interpersonal skills showing initiative and leadership.
5. Ability to work as a member of a multidisciplinary team
6. Commitment to continuing professional development
7. Demonstrate ability to utilise information technology within a variety of applications and willingness to undertake training
8. Current Drivers Licence valid within Australia and willingness to use for work purposes
