The HIP Health Coordinator will be a valued member of the Health Coordination Team. We are a small program providing health coordination in the Hume, Upper Hume and Albury catchment. Our office is based in Wodonga. We are actively seeking candidates who want to be part of this dynamic, innovative team of health professionals. The sucessfull applicant will use their highly developed communication skills, their extensive knowedge of chronic health conditions, service navigation skills and expertise to enhance this well known and respected team.
This health professioal will need to be a team player, highly professional, flexiable, reliable, very experienced, enjoy work and have a passion for helping others.
Employment Type: This is a fixed term part time hours per week, opportunity.
