Job Title: Clinical Care Coordinator
We are seeking a skilled and compassionate Clinical Care Coordinator to join our team. As a key member of our multidisciplinary team, you will be responsible for delivering high-quality clinical care to adults aged 18 years and over who experience mental health concerns.
About the Role
The successful candidate will have strong communication and interpersonal skills, with the ability to engage with people from diverse backgrounds and establish effective relationships with staff, clients, families, and friends.
* Deliver high-quality evidence-based clinical care as a member of the multidisciplinary team
* Conduct intake and assessments, including risk assessments, care treatment planning, care coordination with internal and external services, and provide brief psychological intervention
Requirements
To be considered for this role, you must have:
* Full AHPRA registration as an Occupational Therapist or Mental Health Nurse, General or Provisional AHPRA registration as a Psychologist/Clinical Psychologist OR AASW registration/eligibility as a Social Worker (would suit graduate Social Worker wanting to build skills in the mental health field)
* Flexibility to work afternoons and evenings, weekends, and public holidays
* Knowledge and understanding of the theory underpinning evidence-based interventions and clinical practice, as well as demonstrated excellence in the practical application of such, in the areas of mental health and alcohol and drug use
