We are seeking a dedicated and experienced Clinical Care Coordinator to join our team at our Toogoolawah facility on a part-time basis. About the Role
As Clinical Care Coordinator, you will play a vital role in ensuring our residents receive the highest standard of person-centred clinical care. You will:
* Assess resident needs and develop tailored care plans.
* Lead and support a dedicated care team to deliver safe, high-quality care.
* Maintain accurate clinical documentation and oversee compliance requirements.
* Provide hands-on nursing care, including wound management, medication administration, and monitoring.
* Communicate effectively with residents, families, and health professionals.
* Support infection control measures, clinical audits, and quality improvement initiatives.
* Act as a role model and mentor for nursing and care staff.
About You
We are looking for someone who brings:
* Bachelor of Nursing and current AHPRA registration.
* Experience in residential aged care nursing.
* Strong clinical assessment, documentation, and leadership skills.
* Excellent communication and interpersonal skills.
* Computer literacy, including the use of Microsoft Office and clinical software.
