Job Title: Primary Care Practitioner
The primary objective of this role is to provide high-quality clinical nursing services to doctors and patients.
The practitioner will be responsible for delivering chronic disease management, triage, vaccinations, wound care, pathology collection, health assessments, recall, coordination of the treatment room, cold chain management, and sterilization.
Strong communication and time management skills are essential for this position, as well as a caring and collaborative approach to patient care. The practitioner must also possess strong organizational skills and be able to multitask in a fast-paced environment.
Key Responsibilities:
* Deliver quality clinical nursing services to doctors and patients.
* Provide chronic disease management, triage, vaccinations, wound care, pathology collection, health assessments, recall, coordination of the treatment room, cold chain management, and sterilization.
* Collaborate with other healthcare professionals to ensure effective patient care.
* Develop and implement individualized care plans that meet the unique needs of each patient.
Required Skills and Qualifications:
* Current registration as a registered nurse with AHPRA.
* Experience working in a similar capacity.
* Strong communication and interpersonal skills.
* Ability to work independently and as part of a team.
