This is an exciting opportunity to join our Senior Medical team at Central Coast Health@Home as a GP Visiting Medical Officer (VMO). As GPs, we facilitate hospital/ED avoidance through Hospital in the Home (HITH), Virtual Care and Triage (VCAT) and Aged Care Response (ACRT) teams and with NSW Ambulance.
Central Coast Health @ Home (CCH@H) is seeking GP Visiting Medical Officers (VMOs) to join our Senior Medical team delivering virtual and face-to-face care to reduce hospital admissions.
CCH@H provides hospital-level care at home, supporting patients at risk of hospitalisation via virtual care, home and clinics visits, and residential aged care outreach.
Key responsibilities:
- Support the multidisciplinary care team in triage, admissions, reviews, and clinical management of patients at risk of hospitalisation.
- Manage Hospital in the Home (HITH) patients in Gosford and Wyong clinics and by virtual care.
- Work closely with in-patient teams and EDs to facilitate early discharge and avoid hospital admissions.
- Engage with local GPs, Urgent Care, NSW Ambulance, Residential Aged Care Facilities and assist the Virtual Care and Triage (VCAT) and Aged Care Response (ACRT) teams to reduce ED presentations.
- Participate in a 7-day roster, including evenings and weekends.
Eligible candidates must be registered in NSW and FRACGP, FACRRM, FACEM or FACP.
