Role Overview
The NDIS Care Coordinator plays a pivotal role in ensuring the safe and timely discharge of National Disability Insurance Scheme participants from hospital to the community.
Key Responsibilities:
* Coordinate and enable smooth transitions for NDIS participants.
* Collaborate with clinical teams, NDIA planners, and community providers to achieve positive outcomes.
* Develop and maintain relationships between health services and community providers.
* Support effective referrals to NDIS Hospital Liaison Officers.
* Monitor participant progress and escalate delayed processes.
* Report inpatient delayed discharge data to relevant authorities.
