Clinical care partners play a pivotal role in delivering high-quality patient-centred care. They collaborate with healthcare teams to address the unique needs of patients and their families, providing comprehensive support during client contact.
The job involves travel within designated areas and regular office days. Key responsibilities include:
* Providing empathetic care and support to patients and their families
* Triage and coordination of care management services for short-term pathways such as end-of-life and restorative care
* Monitoring clinical indicators in real-time to identify risks and prevent deterioration
* Collaborating with multidisciplinary teams to plan and evaluate patient care, ensuring high satisfaction and proactive issue resolution
To excel in this position, you will require:
* A degree in nursing or a related field (Bachelor of Nursing) with a minimum of 3 years of experience
* Current registration as a Registered Nurse with AHPRA
* In-depth knowledge of the Aged Care Legislation and Quality Standards
* Strong clinical expertise and understanding of age-related health issues
* Experience in care management, including assessment, care planning, goal writing, and reviewing
* Effective coaching and training skills, with the ability to influence positive change
* Excellent conflict resolution and emotional intelligence skills
* A customer-centric approach to service delivery
* Ability to work independently and as part of a team
* Proficiency in digital literacy, including Microsoft Outlook and Excel
* Demonstrated knowledge of workplace health and safety
