At integratedliving, our Allied Health Services provide direct and virtual services to promote and improve client health and wellbeing outcomes, consistent with the integratedliving signature experience.
What You'll Do:
As part of a multidisciplinary team, you will:
* Conduct comprehensive assessments and develop evidence-based care plans tailored to each client's needs
* Provide direct and virtual Social Work services, including interventions, therapy, and support for clients and their carers
* Facilitate access to Telehealth and other specialised services or equipment in clients' homes
* Collaborate with case managers, health partners, and community stakeholders to ensure holistic care
* Deliver health and wellbeing education through programs, workshops, and community forums
* Participate in case conferences, review client progress, and adapt care plans as needed
* Support continuous improvement, compliance, and quality in all aspects of service delivery
What We're Looking For:
* Bachelor's degree or higher in Social Work
* Accredited Social Worker with AASW membership and accreditation
* Experience in community services or comparable sectors, ideally within multidisciplinary teams
* Strong communication and interpersonal skills, with the ability to engage a variety of stakeholders
* Well-developed IT skills and experience using client management software
* Commitment to continuous professional development and best-practice Social Work
* Satisfactory Police Check & WWCC/WWVP/Blue Card
* Current Driver's Licence
* Current First Aid & CPR
