Role Purpose
The role will be responsible for designing, leading, and governing Pallicare's clinical model of care. This role is foundational to establishing clinical credibility, safety, and scalability across all service settings.
You will define how care is delivered across home, hospital, and community environments, ensuring clinical excellence, governance compliance, and seamless coordination of services.
This is a senior leadership role for a clinician who can operate at both strategic and operational levels.
Key Responsibilities
* Establish and lead Pallicare's clinical governance framework, ensuring safety, compliance, and clinical accountability across all services
* Design and oversee integrated care pathways spanning palliative care, advanced illness, hospital-in-the-home, and end-of-life support
* Define clinical standards, scope of practice, triage criteria, and escalation protocols across home, hospital, and community settings
* Provide senior clinical leadership and decision‐making oversight for complex and high‐risk cases
* Ensure continuity and coordination of care across multidisciplinary teams and external healthcare providers
* Develop and maintain clinical partnerships with hospitals, GPs, specialists, and allied health networks
* Support the translation of the care model into scalable operational systems and workforce structures
* Drive continuous improvement in clinical quality, patient outcomes, and service delivery standards
About You
* Clinical qualification in Nursing, Medicine, or Allied Health (e.g. Social Work, Psychology, Occupational Therapy)
* Specialisation or strong experience in palliative care, end‐of‐life care, or advanced illness management
* Experience in hospital, community health, or home‐based care delivery
* Background in hospital‐in‐the‐home (HITH), aged care, or complex chronic disease management
* Proven experience working within multidisciplinary healthcare teams
* Exposure to clinical governance, quality, and risk management frameworks
* Experience coordinating care across hospitals, GPs, specialists, and community providers
* Ability to operate in complex, high‐acuity clinical environments
* Experience in leadership or senior clinical decision‐making roles
* Strong understanding of patient‐centred and integrated care models
