Monash Care Disability Services is an NDIS-registered provider delivering high-quality, person-centred supports across Victoria and Western Australia. In Kalgoorlie, we operate Supported Independent Living (SIL) homes and provide community and drop-in supports for participants with complex needs.
As our services continue to grow in the Goldfields region, we are seeking a hands-on Service Coordinator to lead day-to-day operations and support our local team.

About the Role
As a Service Coordinator, you will play a key leadership role in ensuring safe, high-quality and compliant service delivery. This is a hands-on position that combines coordination, staff supervision, and staff training to maintain strong frontline engagement and service quality.
You will work closely with participants, families, support workers, allied health professionals and management to ensure supports are delivered in line with NDIS plans and organisational standards.

Key Responsibilities
* Coordinate day-to-day operations of SIL houses and community supports
* Supervise, support and mentor support workers
* Manage rosters for 24/7 active support services
* Ensure compliance with NDIS Practice Standards and organisational policies
* Oversee documentation including progress notes, incident reports and medication records
* Liaise with families, guardians, support coordinators and allied health professionals
* Support participant onboarding, reviews and service planning
* Identify and manage risks, incidents and service improvements

About You
You are a motivated, reliable and people-focused professional who thrives in a regional environment and enjoys being involved on the ground.
Essential Requirements:
* Certificate IV or higher in Disability, Community Services, Mental Health or similar
* Minimum 2 years' experience in disability or community services or similar recommended
* Experience supporting participants with complex needs or behaviours of concern
* Strong understanding of SIL operations and NDIS requirements
* Current WA Driver's Licence
* Willingness to complete hands-on support worker shifts
Desirable:
* Experience in staff supervision or service coordination
* Medication administration competency
* Behaviour support or restrictive practice experience
