The Role:
Supporting an all female group home in Hornsby, we are seeking passionate, caring Disability Support Practitioner to join the team and make a meaningful contribution to our clients' lives. Responsibilities include but are not limited to:
- Supporting clients to develop and maintain independence, and advocate for the clients and their individual needs.
- Delivering personalized support by assisting clients at mealtimes, personal care, administrating medication, manual handling using a variety equipment and aids.
- Transporting clients in our wheelchair accessible vans and pool cars and community access.
- Monitoring the well-being of clients and reporting on their progress.
- Being part of a fun and dynamic team.
