About the Opportunity
The Onboarding Care Connector is a key role within the Onboarding and Service Development team, forming part of a centralised hub that manages the onboarding of Home and Community Care clients.
Acting as a primary point of contact for new enquiries and referrals, this role provides responsive, high-quality information, coordination and administrative support throughout the customer sales and onboarding journey.
Working closely with internal stakeholders, the Onboarding Care Connector ensures referrals are triaged efficiently, onboarding activities are coordinated effectively, and accurate data is maintained across CRM and My Aged Care systems to support smooth service commencement and business growth.
About You
You are organised, proactive and customer-focused, with strong relationship‑building skills and a genuine commitment to supporting older people. You thrive in a fast‑paced environment, balance competing priorities with accuracy and professionalism, and deliver empathetic, high‑quality service at every interaction.
You are confident working with digital systems and enjoy collaborating with stakeholders to ensure a seamless onboarding experience from first enquiry through to service commencement.
Essential Criteria
* Demonstrated customer service and relationship‑building skills
* Understanding of care coordination, care management and care planning
* Proficiency in using CRM systems, digital platforms and client management systems
* National Police Check
* Current unrestricted South Australian Driver’s Licence
Desirable
* Relevant qualifications in business, office administration, sales management and/or aged or community care
