Support Role
We deliver bespoke support services empowering customers to build independence and achieve goals.
This role is designed to work in partnership with individuals living with disability, supporting them to live within the community in a way that they choose.
The key responsibilities of this position include:
* Collaborating with customers to develop individualised person-centered support plans.
* Providing support to customers to enhance their independence by assisting with skill development, transport, social activities, finances, appointments, cooking, medication and personal care.
* Encouraging customers to participate in activities of their choice and promote community engagement.
* Maintaining accurate customer records using online systems and electronic databases.
* Developing and maintaining professional relationships with customers, families, caregivers and stakeholders.
* Exploring innovative ways to improve the recovery journey and overall customer experience.
Candidates are expected to have a minimum Cert III in Disability, mental health, Peer Work or equivalent, and hold a valid NSW Drivers License (Class C).
