Southern Cross Care Queensland has an opportunity for a part-time Clinical Funding Officer to join the clinical team at Oasis Aged Care Home in Mildura. In this part-time role you will be guaranteed to work a minimum of 30 hours per fortnight. If you are a nurse there may be potential to pick up extra nursing shifts.

About the role

In this role, you will provide additional site-specific clinical funding support to the home for the Australian National Aged Care Classifications (AN-ACC) classifications, with the support of the SCCQ Clinical Funding team.

Responsibilities

* Liaise with carers and other clinical staff to ensure documentation of resident care needs and time spent supports funding classifications and appropriate staffing reflective of residents' care needs.
* Assist and support staff in ensuring they possess resources and knowledge of the systems and raise concerns with the Clinical Manager.
* Upskill Residential Aged Care staff in funding processes by providing training to relevant roles (including RN/EN/PCW/Lifestyle Assistants and Pastoral Carers) as required at scheduled sessions agreed by CM/FM.
* Provide ongoing education through toolbox sessions at shift change to increase awareness and incorporate into induction programs for new employees.
* Participate in monitoring resident conditions, clinical meetings, handovers and department meetings to identify potential reclassification opportunities; attend HODs meetings and liaise with staff to identify changes needing screening.
* Monitor and track resident data including movements, Medicare events and AN-ACC lodgments. Participate in the admission process including pre-screening, funding predictions, and development of care plans and communications with the clinical team.
* Review assessments to ensure congruence with care plans and support AN-ACC classification; this includes pre-admission, new admission, and reclassification screening.
* Deliver high-quality documentation, reporting and tracking; monitor occupancy, entries and departures.
* Complete pre-screening, new admission and reclassification forms with required data to support forecasts.
* Complete allocated KPIs for resident screening and reclassification requests to support site KPIs for PRPD.
* Engage with internal stakeholders, including attendance at HODS and AN-ACC meetings, and maintain clear communication with the Clinical Funding Manager.
* Perform data analysis and reporting for AMO visits to conduct assessments and prepare staff for interviews.

Qualifications and requirements

* Experience in a care or clinical role within a Residential Aged Care Facility
* Knowledge of AN-ACC tools, AN-ACC funding guides and processes in a Residential Aged Care Facility
* Proven ability to quickly learn diverse software systems and a working knowledge of Microsoft Office
* Ability to work as part of a team and with minimal supervision
* Attention to detail and strong customer service skills
* Strong written and verbal communication skills
