* Join a dedicated multidisciplinary team supporting older people with complex needs to stay safe, well and independent at home. Manage short term caseloads, build tailored care plans and deliver holistic, person-centred support across community settings.
* Deliver short term case management for complex older clients.
* Assess client needs and plan tailored care and make appropriate onward referrals.
* Build links with families, carers and health teams.
* Advocate for client choice, safety and autonomy.
* Support discharge and avoid hospital admission.
About You
* You are a compassionate clinician who enjoys working with older people and delivering holistic, person-centred care in the community.
* Social Work degree or AHPRA registered OT
* Experience with complex older clients and case management
* Strong communication and teamwork skills
* Ability to work independently and prioritise tasks
* Knowledge of aged care and community care
