We are seeking a passionate and skilled leader to join our team as a Service Team Leader. This is a dynamic role that requires strong communication skills, the ability to work independently, and a commitment to delivering high-quality care to our clients.
About the Role
* As a Service Team Leader, you will be responsible for guiding a team of Home Care Workers to deliver person-centered care that upholds dignity, independence, and quality of life.
* You will work closely with clients, their families, and external providers to tailor support and foster strong, trusted relationships.
* Your key responsibilities will include assessing client needs, developing and reviewing care plans, coordinating services, advocating and educating clients, budget oversight, and relationship building.
* You will also be responsible for maintaining accurate documentation, upholding aged care standards, and proactively managing risks.
Requirements
* Qualifications: Currently holds a Certificate III in Individual Support or similar, Medication competency, First Aid and CPR qualification, Police Check, National Personal Insolvency Check, and a current driver's license.
* Skills: Strong communication skills, highly organized with advanced problem-solving skills, understanding of Home Care Packages, Support at Home Packages, and Commonwealth Home Support Program.
