Case Manager – Recovery & Support
A State Government Agency is seeking a Case Manager – Recovery & Support based in Geelong. You'll manage a portfolio of injured individuals and will be on supporting recovery and return-to-life outcomes through active engagement with individuals, their networks, and service providers.
Responsibilities
Coordinate recovery plans and manage a caseload of complex cases
Provide empathetic support and proactive communication with injured individuals
Engage with support networks, service providers, and internal stakeholders
Identify services and programs that improve recovery and social outcomes
About You
2+ years' case management experience in personal injury, health, or disability
Comfortable supporting sensitive and complex cases
Understanding of recovery models, support services, and return-to-work strategies
Tertiary qualifications in allied health, rehabilitation, or related fields are highly desirable
Previous experience with Novus and Acction highly regarded
