Position: Care Coordinator - Hospital Admission Risk Program (HARP) * Employment Type: Ongoing Full Time and Part Time opportunities available (Please specified your preferred hours within your application) * Location: Warrnambool Campus * Classification: Community Health Nurse * Salary: $110,614.40 pro rata, plus superannuation About the role Are you a skilled and compassionate Registered Nurse looking to make a genuine difference in your community? South West Healthcare (SWH) is seeking an experienced nurse (minimum five years' experience in acute and/or community healthcare) to join our Hospital Admission Risk Program (HARP) as a Care Coordinator. In this role, you will: * Support individuals with chronic and complex health conditions to remain well and independent in the community. * Proactively reduce avoidable hospital admissions by coordinating care, promoting self-management, and improving patient outcomes. * Work closely with GPs, specialists, community health services, aged care providers and acute services to deliver seamless, patient-centred care. What you bring * Current registration with AHPRA as a Registered Nurse. * Minimum of five years' clinical experience in acute and/or community settings. * Demonstrated experience in care coordination, chronic disease management, or case management. * Strong understanding of healthcare systems, referral pathways, and multidisciplinary collaboration. * Excellent communication, problem-solving, and organisational skills. * Ability to work independently and as part of a dynamic team.
