Key Responsibilities:
* Assess and review client and carer needs in partnership with them
* Coordinate timely referrals and service delivery within program guidelines
* Maintain accurate client records, documentation, and reporting
* Support carers through advocacy, information, and conflict resolution
* Communicate effectively with subcontractors and service providers
* Participate in team duties and facilitate group events
