About The Role
Position 1 - Assertive Outreach Nurse – Min 32 hours to 38 hrs/wk – Tweed Community Health
Position 2 - Assertive Outreach Nurse – Min 32 hours to 38 hrs/wk – Byron LGA
The Clinical Nurse Specialist Grade 2 (CNS2) coordinates and delivers high quality nursing care to patients/clients in accordance with the Nursing and Midwifery Board of Australia’s National Standards of Practice for Registered Nurses, criteria for CNS2 as outlined in the Public Hospital Nurses and Midwives (State) Award.
The aim of the CNS2 Assertive Outreach Nurse is to facilitate access to relevant services to improve the health and wellbeing of community members who due to their personal life circumstances and/or reduced social economic status have difficulty accessing appropriate health care.
Specifically, the target population includes individuals who are experiencing homelessness or other socioeconomic disadvantage/isolation and/or those who attend outreach services (such as soup kitchens) aimed at people who are in need of physical, social and economic support.
Working in collaboration with other healthcare providers and relevant government and non-government agencies to ensure that care is well coordinated and that client needs are met.
