Responsibilities
* Deliver high-quality clinical case coordination, assessments, and treatment interventions within a strong clinical governance framework.
* Work collaboratively with a multidisciplinary team to ensure seamless, person-centred care for clients.
* Improve access to primary health services for CALD communities by addressing local service gaps across Northern Sydney.
* Identify health needs and develop targeted intervention plans that make a real difference in client outcomes.
* Provide consultations, health education, and timely referrals to primary and allied health services both onsite and in the community
* Build strong partnerships with community providers, allied health professionals, and GPs to deliver coordinated, continuous care.
* Lead and participate in community events and initiatives that promote health awareness and strengthen connections within CALD communities.
