What you'll do :
* Deliver nursing procedures and clinical assessments
* Develop and coordinate care plans tailored to clients needs
* Manage and monitor ongoing care in collaboration with families & the care team
* Build strong supportive relationships with clients in their homes
What you'll bring :
* Current AHPRA RN registration experience in community‑based nursing
* Strong interpersonal problem‑solving & communication skills
* Ability to work well within a multi‑disciplinary team
* Current drivers licence registered vehicle
* Valid working rights in Australia
* National Police Check NDIS Worker Check (or willingness to obtain)
