Adult Community Services play a central role in supporting adults with complex, acute, and long‐term health needs, enabling timely assessment, treatment, and rehabilitation within community settings. This post offers an exciting opportunity for a Registrar‐grade doctor to develop advanced skills in community medicine, admission avoidance, and Hospital at Home care, while working within a supportive multidisciplinary and educational environment. The role is suitable for a GP Registrar (ST3), seeking experience in integrated care, system working, and service development aligned with future consultant or senior clinician roles.
Main duties of the job
* Senior clinical input to community teams
* Supporting patients to receive safe and effective care in their own homes or other community settings wherever clinically appropriate
* The role involves close collaboration with primary care networks
* Ensure smooth transitions across care settings
* Promote timely alternatives to hospital admission
