Home Care Manager

Full Time

Christian Homes Tasmania Ltd. (CHT) is a not-for-profit provider of aged care services including residential care, community care and independent living housing options. CHT is committed to excellence in all aspects of care and services.

We are seeking a Home Care Manager to lead and grow our Home Care services while ensuring high quality, person-centred care for clients living in the community.

Purpose of the Role

This is a key leadership position within our organisation, responsible for overseeing the delivery of home care services including Support at Home Packages, Department of Veterans’ Affairs packages and fee-for-service arrangements. The role provides operational leadership to Home Care Assistants and works collaboratively with internal teams, including finance and administration, to support client onboarding, service agreements and sustainable service delivery.

Key responsibilities include:
* Overseeing the coordination and delivery of high-quality home care services

* Leading, mentoring and supporting the Home Care team

* Ensuring services comply with aged care legislation, regulatory requirements and quality standards

* Supporting client onboarding, service agreements and care coordination

* Managing workforce performance, recruitment and team development

* Responding to client and family enquiries or concerns in a professional and timely manner

* Ensuring accurate documentation and reporting through electronic care management systems

* Maintaining compliance with My Aged Care and Services Australia requirements

* Contributing to continuous improvement and organisational objectives

* Participating in an on-call roster for after-hours client support

The successful candidate will demonstrate:
* Strong leadership and people management skills

* Excellent communication and relationship-building abilities

* Strong organisational and prioritisation skills

* Ability to work in a fast-paced environment and manage competing priorities

* Proven ability to successfully lead a team and mentor team members

* Well-developed interpersonal and communication skills including conflict resolution and an ability to build positive relationship with colleagues, team members and external stakeholders

* Knowledge of aged care legislation and regulatory requirements (or the ability to quickly develop this knowledge)

Essential requirements:
* Minimum 3 years’ experience in a leadership role

* Valid and satisfactory NDIS Worker Screening Check (with NDIS endorsement) or willingness to obtain

* Valid Australian work entitlements

* Valid Australian driver's license

Desirable
* Experience in the health care or community services industry

* Qualification in a health-related discipline
