Job Summary:
We are seeking a skilled Registered Nurse to join our Home and Community Care team in the Perth metro area. As a key member of our multidisciplinary team, you will be responsible for providing high-quality care to customers in their own homes.

Key Responsibilities:
* Conduct clinical nursing assessments to identify customer needs and preferences
* Develop and implement individualized care plans that promote customer independence and well-being
* Liaise with healthcare professionals, case managers, and other stakeholders to ensure seamless care coordination
* Participate in multidisciplinary team meetings to share knowledge and best practices
* Ensure compliance with organizational standards and guidelines
Requirements:
* Current registration with AHPRA as a Registered Nurse
* Strong communication and interpersonal skills
* Ability to work autonomously and as part of a team
* Competency in clinical assessment and care planning
* Cultural awareness and sensitivity
* Reliable vehicle and current driver's license
