About the Role
As a Disability Services Planner, you will be responsible for managing a portfolio of client records, ensuring high-quality and timely support is delivered.
Your primary duties will include:
Developing and reviewing comprehensive care plans based on individual needs and goals.
Managing and prioritizing a demanding workload to meet key service delivery targets.
Maintaining accurate and detailed client records using IT and data management systems.
Engaging with a range of internal and external stakeholders to facilitate positive outcomes.
Utilising your conflict resolution skills to manage difficult conversations and complex situations with tact and empathy.
Conducting data analysis to inform decisions and contribute to service improvement.
What You'll Bring
To be successful in this role, you will need:
Proven experience in the disability or broader community services sector.
Exceptional communication skills, both written and verbal, with a talent for stakeholder engagement.
Strong IT competency and the ability to work effectively with client record systems and data.
The ability to work both independently and as part of a dynamic team.
A proactive approach to problem-solving and a high level of attention to detail.
Requirement - Australian citizenship required for being eligible for the role.
