Case Manager – Next Steps (Intensive Tenancy Support)

Part time (0.9 FTE), Fixed term contract | Location: Elizabeth Park

Collaborative and client focused team | Experienced and supportive leadership

Support young people transitioning from care to build stable, independent futures.

The Role

This is a newly created role within Centacare Domestic violence unit. We are looking for a dedicated and proactive Case Manager to join our Next Steps program, supporting young people aged 16 to 21 as they transition from residential care into independent living.

In this role you will support young people who are facing complex challenges, including trauma, housing instability, and limited support networks. You will focus on providing intensive tenancy and housing support, alongside therapeutic, trauma-informed care to help young people build stability, independence, and resilience.

Key responsibilities
* Deliver intensive tenancy-focused case management, supporting young people at risk of eviction or housing instability
* Assist young people to establish and sustain tenancies, including social, community, and private housing
* Support practical skills such as budgeting, rent management, property care, and understanding tenancy rights
* Build and maintain strong relationships with housing providers, landlords, and support services
* Collaborate with child protection, health, education, and community services to provide holistic support
* Maintain accurate case notes, reports, and data, and contribute to continuous program improvement

This position is funded on a fixed-term opportunity until 30 June 2027 (possibility of extension subject to funding).

What you need to bring Essential
* Degree in Social Work or related field, or significant relevant experience
* Experience working with young people with complex needs
* Understanding of child protection frameworks and responsibilities
* Strong case management and report writing skills
* Valid DHS Working With Children Check
* Current Driver's Licence
Desirable
* Experience working with young people transitioning from care systems
* Established relationships and strong sector knowledge within the housing sector
* Understanding of housing and tenancy systems
* Experience working with Aboriginal and Torres Strait Islander young people
* Experience in trauma-informed or therapeutic support
* Experience using Penelope or similar client management systems
* Experience in outreach or mobile service delivery
Special Conditions
* Participation in an on-call roster
* Some work outside standard business hours
* Use of fleet vehicle or personal vehicle (grey fleet)
