Clinical Aged Care Assessor - NSW 3 Illawarra
Location: Nowra and surrounding areas
Hours: Monday to Friday, part-time 3 days per week; flexible hours and no shift work
* Pay rate – $54.47 per hour + superannuation

We're expanding our Aged Care Assessment Services (ACAS) team with opportunities across metropolitan and regional locations. This role involves local travel within the region and offers flexibility and autonomy. We have an exciting opportunity for a Clinical Aged Care Assessor to join the team.
What will your role look like
You will be conducting assessments to identify and meet the complex needs of clients and refer to appropriate aged care services under the Aged Care Act.
Day To Day This Includes
* Collaborating with clients, families and multidisciplinary teams as required
* Working with colleagues to promote a supportive and positive team culture
* Conducting accurate aged care assessments using the Integrated Assessment Tool (IAT) to determine client needs and eligibility for home support services
* Reviewing support plans to meet changing client needs
* Working autonomously and as part of a team to deliver to KPIs within identified timeframes
* Maintaining currency of knowledge relating to funded and non-funded aged care services
* Assisting with continuous improvement to evaluate and enhance assessment processes and outcomes
To Be Successful In This Role You'll Need
* Tertiary Qualification in a health-related discipline (e.g., Registered Nurse, Medical Officer, Occupational Therapist, Physiotherapist or Social Worker)
* Current unrestricted registration with AHPRA
* Australian Driver's license and access to a registered, insured vehicle
* Minimum of one year experience delivering services in Australian aged care settings or with older persons, including those living with dementia
* Willingness to apply for a Police Check
