Job Description

Key Responsibilities
* Comprehensive Drug & Alcohol Assessments: Conduct thorough assessments in collaboration with mental health clinicians, providing integrated discharge planning for complex patients and carers with dual diagnosis.
* Cross-Functional Collaboration: Work alongside mental health clinicians, peer workers, and community stakeholders across mental health, drug, and alcohol services to ensure coordinated care and support are in place before discharge, aiming to improve patient outcomes.
* Led Delivery of Clinical Practice: Lead the delivery of evidence-based clinical practice, provide education and management guidance to healthcare professionals, contribute to research, and offer professional leadership.
Required Skills and Qualifications
This role requires advanced nursing skills, mental health knowledge, and evidence-based practice. You must be able to work effectively in a team environment and possess strong communication and interpersonal skills.
