About This Role:
The Assertive Community Treatment Team at Fremantle Hospital Mental Health Service is based on the assertive community treatment model, a community psychiatric rehabilitation approach characterized by intensive contact with patients in their own environment to help them build/rebuild their lives.
This role involves providing clinical and professional expertise to ensure comprehensive evidence-based nursing care is delivered to all patients. As part of a multidisciplinary team, you will provide a holistic approach within the Recovery Model, facilitating patient safety and quality of care at all times.
You will work alongside patients, carers, and other appropriate services/agencies to optimize care and service provision. This role requires consideration of the Nursing and Midwifery Board's Nursing Practice Decision Flowchart.

Skills and Qualifications:
To be successful in this role, you will need to possess excellent communication and interpersonal skills, as well as the ability to work effectively in a multidisciplinary team environment. You should also have a strong understanding of the principles of recovery-focused care and be able to demonstrate your commitment to delivering high-quality patient care.
A relevant qualification in nursing and current registration with the Australian Health Practitioner Regulation Agency (AHPRA) are essential requirements for this role. Experience working in mental health settings would be highly regarded.
