Your Opportunity Case Manager to join our Reconnect team in Whyalla .In this rewarding role, you will work closely with young people aged 12-18, providing tailored support to help them overcome barriers and achieve their goals.

You'll be part of a community-based program that offers holistic services including employment assistance, mental health support, life skills development, advocacy, and family mediation.

About Reconnect program Reconnect is a free, community-based initiative aimed at preventing youth homelessness.

The program supports young people who are at risk of or experiencing homelessness by helping them reconnect with education, employment, family, and community Your Key Responsibilities Deliver case management and support to young people aged 12-18 who are at risk of homelessness or disengaged from education.

Develop and implement individualised support plans in collaboration with clients.

Connect clients with relevant services, programs, and community resources.

Build strong relationships with schools, training providers, employers, and local organisations.

Monitor progress, maintain accurate records, and report on outcomes.

Advocate for clients and empower them to build confidence and resilience.

Requirements for Success Certificate IV in Youth Work, Community Services, or a related field (or equivalent experience).

Experience working with young people or vulnerable cohorts in a support or case management role.

Strong communication, interpersonal, and problem-solving skills.

Ability to work independently and collaboratively within a multidisciplinary team.

Knowledge of local education, training, and employment pathways is highly desirableBefore starting work with us, you will need to undertake a national police history check, qualifications, and referee checks, and have a clearance to work with children/vulnerable people.
