DFP Recruitment is seeking a Clinical Care Coordinator to join our Federal Government clients team on a long term assignment (up to months).
This role is full time hours (Mon - Fri) and will be based in Winnellie, NT.The position of Clinical Care Coordinator entails working collaboratively with clients and internal and external stakeholders to enhance the ability to identify and respond to the needs of vulnerable and at-risk clients.
The Clinical Care Coordinator will take a strengths-based, recover approach to supporting vulnerable and/or at-risk clients to enhance wellbeing and participation in their community through community engagement, collaborative recovery work, case coordination and complex case management.
The role requires you to:Liaise and form partnerships with various internal and external stakeholdersDirect provision of high quality, evidence informed clinical services including intake, assessment and referral, complex case management, and case coordinationRisk assessment and interventionEvidence based individual, couple and family counsellingMaintain clinical records, collating and analysing data from client records & preparing reports as requiredWork systemically with key internal and external stakeholders to influence a recovery approach within a stepped care model for vulnerable or at-risk veteran and defence force communities and familiesParticipate in community development and service promotion activitiesContribute to the development and delivery of group programsAbout you:Degree qualified Registered Mental Health Nurse, Psychologist, Social Worker or Occupational Therapist with two years of Mental Health experienceFull registration with AHPRA or AASWWorking with Children (Blue Card) or Working with Vulnerable People (Yellow Card)An Australian Citizen and willing to undertake a National Police Clearance.In return you will receive:An attractive hourly rate of $ per hour + super Monday to Friday, .5 hours per weekExtensive training and ongoing support & development month contract with a potential extensions To express your interest in this position please submit your application as soon as possible.
