Job Description:
The Clinical Care Manager is responsible for overseeing and guiding a team of nurses working in the facility. This role requires promoting values that reflect the organization's mission, demonstrating emotional maturity, cultural awareness, and flexibility when interacting with individuals and situations. It also involves leading by example and influencing others to create a supportive work environment.
Key Responsibilities:
1. Provide clinical leadership to ensure high-quality care for all residents in the facility.
2. Regularly examine and review clinical incidents, audit data, and clinical indicators, undertaking root cause analysis and reporting as needed.
3. Collaborate with the nursing team and Facility Manager to facilitate monthly clinical incident and analysis reports.
4. Provide oversight of the High-Risk Resident Framework to ensure residents identified as having high-impact, high-prevalence risks receive support and care through a multi-disciplinary approach.
5. Ensure legal and ethical compliance of restrictive practices in line with Aged Care Quality Standards, obtaining informed consent from residents or their representatives, and providing staff support in minimizing the use of restrictive practices.
6. Under the direction of the Facility Manager and Executive Manager Clinical Governance, lead infectious illness outbreak responses, collecting resident documentation and line listings during outbreaks.
7. Maintain accurate and up-to-date care plans, developing schedules for completion and monitoring compliance.
8. Practice according to the Nursing and Midwifery Board of Australia's code of ethics, conduct, and standards of practice.
9. Ensure resident health aligns with advance care directives.
10. Develop and implement robust systems and processes to provide efficient service delivery to residents and consumers.
11. Work collaboratively with the clinical team and Facility Manager to assess and provide residents with the necessary resources according to the Australian National Aged Care Classification (AN ACC) funding model.
12. Provide excellence in antimicrobial stewardship to improve resident outcomes.
13. Collect feedback from residents and families to ensure timely information, support, and consultation regarding care needs.
14. Supervise and support students undertaking nursing placements.
15. Participate in an after-hours on-call roster, nurturing talent and engaging in succession planning, coaching, and mentoring of multidisciplinary teams.
16. Identify and implement ways to challenge and develop the team creatively.
17. Support direct reports in prioritizing, delegating, and effectively managing allocated workloads.
18. Provide guidance to the clinical team, including house companions, in delivering holistic care to residents in the small house model.
19. Monitor and manage clinical supplies ordering processes, establishing and maintaining imprest stock levels, and minimizing stock wastage.
20. Utilize data and industry trends to inform decision-making for purchasing clinical supplies.
21. Apply risk management principles to ensure risks to persons in the work environment are effectively managed.
22. Contribute to a safe workplace through adherence to health, safety, and wellbeing policies and procedures.
23. Report and manage health, safety, and wellbeing concerns in a timely manner.
24. Promote a positive, friendly, and happy approach in the workplace to enhance health and wellbeing.
25. Provide emotional support to consumers and employees.
