The NDIS offers assistance to people with a broad range of physical, intellectual and psychosocial impairments. Sometimes, navigating and coordinating this funding can be tricky; and for people who have complex challenges, it can be even more difficult to maximise all the services available to them. This is where a Mercy Community Support Coordinator comes in!
Support Coordinators work directly with people and broader networks, connecting them together in order for a person to implement their NDIS Plan effectively, and ultimately achieving their goals and aspirations and to go about day-to-day life.
We currently have an opportunity for a Support Coordinator to join the Mercy team in Toowoomba!.
**Day to day you will**:
- Support and communicate participants, their families and their formal/informal support network to understand their NDIS plan;
- Identify the most appropriate services within the community to suit an individual's needs;
- Support participants to communicate their goals and aspirations to their stakeholder team and the NDIA;
- Attend home and community visits with participants;
- Undertake general case management of participant portfolio's; and
- Manage and monitor individual NDIS Plan budgets.
Some of these points sound complicated, however if you are: an organised person, have excellent communication skills, an eye for detail, and the ability to think outside the box - you will excel with all of the training that you will be provided with!
