Key Responsibilities:
* Provide on-ground support and implement relationship-based strategies by modelling and coaching clients, carers, and care staff.
* Collaborate with peers to develop aged care staff and carer skills, expertise, and knowledge in understanding person-centred dementia care.
* Support the implementation of recommendation plans on the ground by drawing on modelling and coaching techniques.
* Facilitate and support trial and implementation of brokerage items with clients and their carers.
Requirements:
* Strong organisational abilities, effective time management, and prioritisation in a busy work environment.
* Ability to work independently and as part of a team, commitment to customer service principles.
* Current driver's licence and safe driving record, willingness to travel intrastate and interstate.
* Qualification in relevant field, such as Allied Health assistant, Diversional Therapy, Enrolled Nursing or Aged Care.
