With a commitment to delivering exceptional, world-class, person-centric disability services in Perth, WA, we have experienced rapid growth and are now seeking skilled and enthusiastic Team Leaders to join our team in a full-time capacity.Duties and ResponsibilitiesCollaborate with external stakeholders such as government agencies, community organizations, and other service providers to improve service coordination and referral pathways.Ensure that appropriate quality assurance processes are in place and that service standards are being met.Coordinate, plan, and execute activities, routines, and strategies that support the goals of our participants as identified in their person-centered NDIS plans.Perform participant and house risk assessments in a timely manner.Develop and maintain positive relationships with clients and their families, as well as other stakeholders.Participate in on-call duties on a rotating basis.Ensure the smooth operation of support worker teams by managing attendance, shifts, and replacements.Work in a person-centered approach, valuing and respecting others, and thinking outside the box.Demonstrate strong leadership skills, including staff management and supervision.Maintain strong knowledge of NDIS laws and regulations.Create service agreements, support plans, and progress reports.Maintain high levels of privacy and confidentiality.Represent the company professionally both internally and externally.Experience and QualificationsDiploma in Community Services or relevant experience as a service coordinator.Experience working as a service coordinator in the NDIS sector (preferable).Strong knowledge of NDIS laws and regulations.Clinical knowledge is an advantage.Skilled in managing customer expectations and handling sensitive issues discreetly.High interpersonal and communication skills, with the ability to liaise effectively and negotiate.Proficient in Microsoft Office suite.Problem-solving and influencing skills, with strategic thinking abilities.Current Australian driver's license and car insurance.Excellent time management and prioritization skills.Experience in creating quotes, service agreements, support plans, and progress reports.Excellent oral and written communication skills.Benefits as a valued employeeMeaningful Impact: Make a significant difference in your community.Supportive Environment: Develop valuable skills in a challenging yet supportive setting.Flexibility: Enjoy flexible working conditions.Access to Work Vehicle: Facilitate transport between sites.Dedicated Leadership: Support from a team of leaders.Ongoing Training: Continuous professional development.Employee Assistance Program: Resources for you and your family's well-being.Click the 'APPLY NOW' button to submit your application.
