Where you'll be working:

Based at Toronto Community Health Centre, you'll be part of Westlakes Community Nursing Service, located at 6 James Street, Toronto NSW – right on the picturesque Lake Macquarie. We're a small, friendly, and dedicated team working together to deliver high-quality nursing care to the local community. As part of Integrated Care Services – Greater Newcastle & Lower Hunter, you'll enjoy both professional support and the natural beauty of the lakeside lifestyle.

About the role:

This is a full-time, permanent position working Monday to Friday, 8am to 4.30pm. In this role, you'll provide a wide variety of nursing services, caring for clients in both the community and clinic setting. You'll have the opportunity to work independently while also collaborating closely with other clinicians, clients, and external providers. From wound management and continence care through to palliative support and general nursing assessments, your work will have a meaningful impact on the lives of those you support.

You are an experienced and adaptable nurse who thrives on variety and values teamwork. You're confident working autonomously, yet enjoy building strong professional connections with colleagues, clients, and service providers. Your ability to deliver person-centred care with professionalism and compassion sets you apart.

Requirements:

The successful candidate will demonstrate the ability to provide:

* Wound assessment and clinical management
* Generalist management of PICC lines, portacaths, and clexane
* Palliative care support alongside specialist providers
* General nursing assessments and appropriate referrals
* Continence assessment and clinical management
* Urinary catheter care and clinical management
