ANOTHEN Community Care is a specialised mental health community care provider, and we have openings for experienced community mental health nurses to join our growing team of health care professionals. Our mental health nurses provide ongoing supports to our clients in the community working with a diverse team to see our clients flourish in the community setting.
ANOTHEN Community Care is an NDIS provider with strong links to other stakeholders in the mental health sector to ensure our clients receive the best possible ongoing treatment and supports. Our team of RNs work with the client's support and treatment network to ensure they are connected to and received the treatment and supports they require.
The Role
* Support transition from an acute hospital setting or prevent hospital admissions into a community setting / supported living
* Collaborate with a passionate multidisciplinary team committed to early intervention and recovery
* Work within a multidisciplinary team: Supporting recovery-oriented practice, supporting clinical interventions, actively participating in team, case and handover meetings, enhancing collaboration between team members.
* Develop, monitor and evaluate a plan of care in consultation with clients, carers and the health care team
* Provide case management services by conducting accurate, comprehensive and timely client assessments
* Promote effective therapeutic and professional relationships within the care environment
* Support and education support workers who provide day to day support for our clients
What you will bring to the role
* AHPRA registration
* Minimum 12 months post graduate experience in mental health
* Effective communication skills
* Current clinical competencies
* Nationally Coordinated Criminal History Check (NCCHC) or prepared to obtain one
* NDIS Workers Check – or prepared to obtain one
