Role
We are seeking a dedicated and experienced Social Worker to join our multidisciplinary team. The ideal candidate will work directly with NDIS participants to assess their needs, develop support plans, and connect them with essential community and therapeutic services. This role involves working closely with participants, families, carers, and other professionals to ensure participants receive holistic and coordinated support under the NDIS framework.

About the Role
As a Social Worker at Beyond Health Services, you will:
* Conduct comprehensive psychosocial assessments to identify participant needs and strengths.
* Develop and implement person-centred support plans in collaboration with participants and their support networks.
* Provide emotional support, crisis intervention, and counselling to participants facing behavioural, social, or economic challenges.
* Liaise with external agencies, healthcare providers, and community organisations to coordinate supports and improve access to services.
* Advocate on behalf of participants to ensure their rights, preferences, and goals are respected and prioritised.
* Assist participants in building capacity, fostering independence, and enhancing social and community participation.
* Monitor and review participant progress, adjusting supports as required in line with NDIS goals and outcomes.
* Maintain accurate, detailed, and confidential records in accordance with NDIS Quality and Safeguards Commission standards.
* Uphold Beyond Health Services' values of respect, empowerment, and inclusion in all interactions.
About You
To be successful in this role, you will have:
* A recognised qualification in Social Work (Bachelor's or Master's degree).
* Current registration or eligibility for membership with the Australian Association of Social Workers (AASW).
* Demonstrated experience working within the NDIS or disability sector.
* Strong understanding of the NDIS Practice Standards, Code of Conduct, and participant rights.
* Excellent interpersonal and communication skills, with the ability to engage effectively with people from diverse backgrounds.
* Experience supporting individuals with psychosocial disabilities, behavioural challenges, or complex social needs.
* The ability to work independently, prioritise tasks, and manage complex caseloads.
* High level of empathy, professionalism, and commitment to person-centred practice.
NDIS Compliance Requirements
All applicants must hold:
* A valid NDIS Worker Screening Check.
* A current Working with Children Check (or equivalent for your state/territory).
* Current First Aid and CPR certification.
* A valid driver's licence and reliable vehicle (with comprehensive insurance).
* Proof of Australian working rights.
