About the Role
Step into a pivotal role coordinating patient care across CALHN. You will guide teams, manage patient flow, and collaborate with clinical and external partners to ensure seamless, high-quality care and improved outcomes for every patient.
Duties
• Coordinate patient movement across CALHN and private providers
• Support less experienced nurses and promote team development
• Monitor care progress and maintain continuity across services
• Build strong relationships with internal teams and external partners
• Contribute to projects and initiatives to improve patient flow
About You
You are a confident and skilled nurse who thrives in coordinating patient care, supporting teams and delivering safe, high-quality outcomes across CALHN.
• Hold registration or eligibility with Nursing Board of Australia
• Deliver nursing care in line with professional standards and ethics
• Lead and support less experienced nurses and students
• Work effectively within multidisciplinary healthcare teams
• Communicate clearly, solve problems and manage competing priorities
